- Killed in a car accident on his way to the track for filming of a racing scene in The First Auto (1927).

- Obituaries at the time referred to the youthful-looking Mack as a "well-known screen juvenile" who was 20 years old. In fact, Mack was 31 at the time of his death.
- Killed at Fourth Street and Fairmount Boulevard in Riverside when the car he was driving collided with one driven by Katherine Callahan of Riverside and then overturned. It was the second fatal accident within five days among the Warner Brothers crew filming at the fairgrounds there, with Helen Howard dying March 13, 1927, of injuries sustained in an auto accident of March 11.

- His Stepdaughter Diana Mack (1914-2007) had a short career in films. She was a Busby Berkeley dancer in "Gold Diggers of 1935" and appeared in two other films, then retired after marrying Western-actor Bob Woodward.
