And as the war between Moretti (Michael Rapaport) and Constantine (William Forsythe) escalates, Grace must keep her brother from becoming its next victim.

Constantine offers to clear the son's debt, but in order to do that, he needs help from Al Trapani. But first Trapani needs some help of his own: his girlfriend is sick and needs a doctor. Constantine assigns Grace to take care of Trapani's girlfriend and in the process she discovers a secret that could jeopardize both their lives if it ever comes out. Meanwhile, Olivia and Dr. White deal with the medical mystery of a patient who died in the emergency room, only to come back to life. The episode ends with the drug cartel, upset that Constantine is interfering with their business, making a major move against Constantine - and everyone he knows.
- As she struggles to stay out of a brewing gang war, Grace is tapped to treat Brett's estranged brother following a bicycle accident. Nate turns to his sister after he accidentally jeopardizes a mobster's valuable racehorse.
- The episode begins right where the previous episode ended - a dangerous drug cartel has made a major move against Constantine, and Daniella was caught in the crossfire. With both her mom and Constantine fighting for their lives in Grace's hospital, Grace's mob life and her personal life have finally collided with the most tragic consequences possible. And just when things couldn't get any worse, the Feds show up at the hospital, asking questions about Grace's involvement in the Chicago mob. As the whole world collapses around her, it's up to Grace to step up and negotiate a truce with the drug cartel on behalf of Constantine while simultaneously dealing with long-buried family secrets that have surfaced amidst the chaos.
