- After splitting from her boyfriend, junior doctor Ruby Walker seeks a new start away from England and answers an advert to work at the busy, underfunded Good Karma hospital in India, run by straight-talking Doctor Lydia Fonseca, along with elderly surgeon Ram Nair and arrogant younger Gabriel Varna. Ruby wins her spurs dealing with a difficult pregnancy but is overwhelmed by the volume of patients, including Maggie Smart, who is in the country with husband Paul for their daughter's wedding and has a brain tumour. When the team fails to save two brothers brought in suffering from stab wounds Ruby considers quitting but is dissuaded by Lydia--who is not amused to find that her occasional boyfriend Greg McConnell has donated an ambulance advertising his bar.
- A muscular young man, totally nude and suffering from amnesia, is dragged from the sea and brought to the Good Karma hospital, where he gives no clues to his identity. Ultimately Lydia discovers his name and his secret and, helped by Ruby, makes him face up to his situation. Ruby also visits a village school to give the children their medicals; teacher Anton Karkik asks her for a date, which she declines, and she helps pupil Vijay, who is withdrawn and undersized for her age. This leads Ruby to a confrontation with his single mother and the discovery of an incident in Gabriel's past. Gabriel himself races against time to collect the serum to save a roguish snake-charmer bitten by one of his animals and almost fails, following a road accident, while Maggie decides that she wants to die in India.
- A blast from the past for Dr. Fonesca reunites a father and son, but her diagnosis leads to more separation. An infant is put at risk through good intentions that result in a situation in which people in authority are unwilling to contemplate other options. AJ is caught up in his pretend doctor's life and finally sees the harm that he can do to innocent people who believe in him. Dr. Fonesca, Dr. Varma, and Dr. Walker experience a Holi festival that serves as a reminder to consider the things in life that are truly important.

The latter is jeopardized by a power cut caused by a thunderstorm, requiring emergency surgery which ultimately gains Lydia's approval and an admission from Gabriel.
- Lydia is outraged to find that her former colleague Anila is now operating as a herbal remedist, poaching her patients. Lydia is particularly concerned for bus driver Reyna, especially when her intervention costs Reyna her job, and decides to put things right. Ruby involves Gabriel in discovering why a young boy is stealing from the hospital and they are both caught up with English tourist Sean's problems.
- Ruby sets up an outreach clinic at a distant tea plantation, accompanied by a reluctant Gabriel, but her true motive is to locate her uncle Amir to ask why her father walked out on her and her mother. Answers become clearer thanks to Gabriel, thawing their relationship but another admission comes as a terrible shock to her. Back at the hospital, Lydia is concerned for Neelam Sagar, suffering domestic violence from her policeman husband Sanjay, but when Sanjay is viciously attacked, Lydia is presented with a dilemma.
- Ruby is still reeling from her discoveries at the tea plantation and must decide if she can forgive past mistakes. Mari goes head to head with Lydia when a surrogate mother gives birth in the hospital.
- When a building collapses onto the local market, the hospital is inundated with casualties. Ruby helps Lydia amputate the crushed arm of Mohanan, trapped under rubble, and discovers the reason for the rift with his brother. Ram cares for confused, elderly Sushma, who is concerned for her missing husband, leading him to a further revelation while Greg attempts to shake Paul from his grieving.

When another injured youngster arrives, Ruby discovers the cause of a feud between local fishermen. Lydia answers a call to visit Virginia Mileham, her old mentor and the hospital's founder, who is terminally ill and wants Lydia to help her go swiftly, which leads to Lydia striking a deal with her. Afterwards the staff attend Ram's wedding, where there are admissions from some of the guests.

Her sister's pregnancy leads her back to Good Karma, but she finds that her return is not easy. Meanwhile, Greg's daughter, whom he hasn't seen in three years visits. Will she stick around? And how will Dr Varma react to Ruby's return?
