When Matsunaga throws the Doctor out of the "Bolero", we see him fall on his back. His shirtsleeves are rolled up, so his forearms are bare: his left arm does not make contact with the ground. The next scene is of him cleaning a wound in his left forearm, below the elbow.
When Matsunaga wakes up in Nanae's bed, he has a wet compress on his chest, under the pajama shirt. When he sits up and takes it off, it is on top of his shirt.
Even though the story takes place during the hot summer, breath can be seen coming from characters in several scenes.
