As you see in the rescue boat scene, Nikos/Klaus felt forced to eat the body of his dead son, just to survive. His wife wouldn't allow it, so in desperation he killed his wife.
It's never mentioned, but it's clear after killing her he ate the kid, and maybe also his wife. By doing this he got mad (who wouldn't :-) ) and became a cannibalistic serial killer on this island: the man eater.

The UK version of this scandalous splatter classic by Joe D'Amato was directly taken from the R-Rated version and was not further cut. Besides some violence, there were also some plot scenes cut. This was probably done in order to shorten the movie and tighten the storyline, which happens quite often in the US. In total nearly 9 minutes are missing in the R-Rated/BBFC 18 Version.
