- Julia enters a room where family photographs are seen with blooding pouring out of them. A whole cabinet damages itself with insects crawling around.
- Julia is surrounded by the Cenobites.
- A sheet with women inside it pops in and out of the wall.
- Easily the most disturbing & brutal entry in the series.
- Frightening characters and creatures shown. Along with very psychotic acting mental patients, movie as a whole though is very "cheesy" and kind of "corny" particularly compared to todays standards.
- A mentally ill man hallucinates that bugs and maggots are crawling on his skin. This is shown several times. He is taken advantage off by being offered to cut them off of him with a razor blade which he does for several seconds.

Violence & Gore
- Julia kills a man by having wires slowly slicing his head without the entirety shown.
- Julia takes out Frank's heart.
- Channard (in Cenobite form) kill all of the Cenobties. After they die, they are shown in human form. He slices Pinhead's (human) neck with blood pouring out.
- While Channard tried to observe the mattress, he brings in a mental patient who proceeds to self harm by cutting himself with a razor severely until Julia rises from the mattress and kills him. This scene is not only very bloody and graphic, but may disturb some viewers due to the self harming scene.
- Brief but graphic descriptions of dead bodies covered in flies & insects.
- A man violently cuts his chest and crotch with a razor blade. Blood spreads all over.
Frightening & Intense Scenes
- A man sits on a mattress to mutilate himself with a razor blade in order to resurrect her. This is done in a very gory way by showing him crying in pain as he cuts himself. As his blood spreads all over the mattress, skinless Julia arises from it and grabs him. They both crawl out of the bed, she crawls on the floor and kills him by sucking him.
- Frank returns in his true form.
- Julia shoves her arm into Frank (in muscle form) and takes out his heart.
- A man (in Cenobite form) gets hooked. His tentacles cut the chains off. He kills three of the Cenobites. After they die, it shows them in human form.
- A flashback shows Dr. Channards sadistic tendencies, as a child where he is shown to have killed and mutilated an animal. He is also frequently shown committing unethical, dangerous, and abusive procedures with his patients.
