- the levels contain different types of enemies natural and otherwise. Ecco can attack enemies using a ramming attack, like in previous installments of Ecco The Dolphin. in addition he can also attack enemies using his sonar with a power up found later in the game, there is also a ramming power up which increases his ramming attack damage.
- both Ecco and enemies will flash red when injured. hardly any blood is shown if any at all.
- in one part of the game the player must blind an alien creature by ramming out it's eyes. two eyes in front and one in the back. it takes around 2 times. the eyes will first be bruised and purple and then they will disappear after an additional attack. not extremely graphic however.
- some of the bigger enemies, especially a few bosses will grab Ecco inside their mouth and he will shout in pain as it is implied that he is being chewed.
- there are some smaller enemies who will attack by biting onto Ecco. one type can kill him by slowly leeching his health, and another type will grab onto him and bite him repeatedly until he dies. the environment is also dangerous in of itself, Ecco will die if he cannot breathe air from either the surface or air bubbles underwater.
- there are also anemones, and certain plants, some natural and some unnatural that will damage Ecco if he swims too close to them. one kind of plant will even draw Ecco into itself and the player will lose either half or all of their oxygen.
- the final level involves the player attacking and destroying a giant beating heart, the heart of the foe. in order to complete the level Ecco must use his sonar to grab onto and throw antibodies into the heart, at which point it can be attacked, after several times the heart will disengage from the roof of the chest cavity and will fall to the bottom, the ending scene will then be of Ecco escaping from the body of the foe.

- the game and story can have some dark and scary themes at times. an alien invasion, time travel and the world becoming a dark and dying place. an implied war between dolphins and humankind. dolphins later on have the power to alter and make the world what they want it to be, but it is still not a good place.
- some of the enemies and environments might scare children and even older players.
