Dawn O'Keefe has brown eyes as a child in the first scene, but has blue eyes in the rest of the movie. Although infant eye colour can change from blue to brown with age, a change from brown to blue does not occur.
When Ryan is pouring Dawn a glass of champagne, the foam appears to be filling half the glass. When he hands her the glass, it has instantly dissipated. When he pours his own glass, it takes much longer for the foam to dissipate.
Scenes of the nuclear power plant cooling towers are shown with columns of thick black smoke. Unless the plant is on fire, there will never be anything other than white steam rising from them.
Shortly past the halfway mark, when Dawn starts researching her condition online, there are a number of grammatical and spelling errors, e.g., on the first page: "orthe" instead of "or the," "to information" instead of "for information." When she goes to the first link: "fears castration" instead of "fears of castration," "found aboriginal" instead of "found in aboriginal." Also, sentences are repeated, in entirety or in part.
When Dawn is 'checked' at the clinic, she flees the room screaming after 'biting' off the gynecologist's fingers. As she opens the door she's displaying very visible underwear lines through her surgical gown. Given the antics displayed, she's not supposed to be wearing any underwear.
