- Things are still tense between Wyatt and Sack when they're called to a shooting, and a gang leader doesn't want the victim taken to the hospital. Alice's mother shows up unexpectedly, and Angela and Harper learn things about each other.
- Wyatt's patients include the barrio brother Jorge and Omar, too scared to accuse street gang leader Ignatio, who obviously shoots them. Alice's manic-depressed mother turns up to renew the ties with her daughter after two years, but ends up betraying why Ally 'killed her future with Wyatt'. Harper shows himself in a different light, and literally. Dealer Misha offers Wyatt a way to redeem himself by hustling.—KGF Vissers
- Wyatt Cole (Tom Everett Scott) and John "Sack" Hallon (Omari Hardwick) are still in non-speaking terms when they have to pick a gang member in their ambulance. At the beginning, the gang leader Ignatio (Zak Santiago) refuses to let them take Omar (Christopher Attadia) , who has been shot in a leg and is bleeding like crazy. Wyatt says to Ignatio that he has arrested Omar, and that Ignatio may get a five-year sentence if he stops him, so Ignatio lets Wyatt and Sack go. Omar says that it was an accident while he was cleaning up his gun.
Alice Alden (Elizabeth Reaser) is visited by her mother (Cristine Rose) who is not taking her medication. She doesn't have so many high highs, but she doesn't get the lows either. Amanda still has good reminiscences about Alice's ex-boyfriend, Wyatt. Wyatt had given a gift to Alice, but she didn't have time to open it. Alice is maniac-depressive, and shopping is one of her many escapist excuses. Amanda is living with Alice, but the daughter gets frantic. Alice only wanted to spend time studying, but Wyatt convinced her of making love instead. Wyatt remembers those tiems with love.
Two students try to make amphetamines at the school laboratory, but things go wrong. Misha (Christopher Redman) offers a way for Wyatt to pay his debts off, making another person to lose. Wyatt refuses.
Jorge gets shot by Ignatio. Jorge tells them that, please, if he dies, Wyatt and Sack tell Omar to leave the gang. The paramedics try to convince him of betraying Ignation. Jorge faints. Alice makes more procedures to Amanda. Amanda tells Wyatt that Alice got pregnant with his child. Alice hadn't told Wyatt that she was pregnant. She just left, had an abbortion and came back to split up with Wyatt. Alice says that her mother is hallucinating and inventing lies. Alice gets frantic because Wyatt believed her mother and he has accused her of that.
Ignatio comes to the hospital asking for Jorge. He makes a pass at Tracy Vilar (Angela de la Cruz). There is a fight with Harper Sims, (Michael McMillian), who defends Tracy. They talk about when he was a missionary, knew a girl in Guadalajara, USA and learnt my swearings in Spanish. Alice shouts at her mother because she has told Wyatt the truth about her and her abbortion, but Amanda tells her that she should have been the one to tell Wyatt the trugh.
Wyatt destroys the idol which Sack had cleaned up. He phones Mischa to say that he will do what he wanted. Finally, Alice opened Wyatt's gift, a secret-teller ring he had come across in E-bay. The both of them had the serious conversation they should have maintained some years ago. However, that night, Wyatt was completely drunk at a party. Distraught Alice asks him to get serious, but he can't. Wyatt said that the following day he wasn't drunk: he could have waited until the moment when he was not drunk. Amanda found the positive pregnancy test while cleaning up the toilet. Amanda really wanted to have that grand-daughter, and gets excited and incredibly happy about the news. Amanda starts telling everybody. They already think about girl names, Alisa, Phoebe... but Amanda was so frantic telling everybody by the phone cell, that they had an accident. Alice lost the child in the accident. At the emergency room, they looked for internal injuries, and they found out that the baby was not well.
Wyatt listens attentively, so he doesn't pick up the phone when Mischa phones him. Wyatt and Alice hug each other.
Finally, Wyatt arrives to the poker table which Mischa had prepared. Wyatt remembers when he first saw Alice. He was playing poker as well.
