- When Wanda's appendix ruptures with her parents out of town, Doogie ignores hospital policy to operate on her. This creates tension with Dr. Canfield as well as Wanda.
- As Doogie arrives to pick up Wanda for a movie date, her parents are getting ready to leave for a dinner cruise to Catalina. Wanda comes downstairs, and her dad tells her to be back home before midnight. He tells her it's on the honor system, as their boat won't dock until 2 am. When they leave, Doogie nervously asks Wanda if she could be his girlfriend. Wanda says she'd love to. Relieved and happy, Doogie kisses her before collapsing on the lawn, to Wanda's amusement.
After the movie, Doogie and Wanda are snuggling against each other in the front seat of the car when Wanda complains of a stomachache. Meanwhile, in the back seat, Vinnie grabs Doogie's stethoscope and tongue depressor from his bag and pretends to examine his date Janine. When Vinnie makes a comment about making sure Janine's cavities are in working order, she leaves the car, leaving Doogie and Wanda alone. They share a kiss, and Wanda complains that her stomach is really hurting. Doogie suspects that Wanda may have appendicitis, and calls Janine and a half-undressed Vinnie back to the car, saying he needs to take Wanda to the hospital now.
At Eastman, Doogie rushes Wanda into the emergency room, saying he wants to admit her and get some blood work. The admitting nurse says that as a minor, Wanda needs parental consent to be admitted, but Doogie says that Wanda's parents are out on a cruise and can't be reached at the moment. Doogie tells Nurse Curly that he wants to get someone to examine Wanda, but she says everyone's tied up, as there's been a bad bus accident. Nurse Curly gets another surgeon, Dr. Fowler, on the phone, and as Doogie talks to him he realizes that he'll have to be Wanda's attending physician. He asks her if she's pregnant, and she says no. He then asks her if she's sexually active, saying he has to ask as part of the procedure. Wanda says no.
Doogie tells Wanda he'll have to do a pelvic examination on her to confirm that she has appendicitis, saying there's no time to do an ultrasound, and a pelvic is the only way he can confirm the diagnosis. Wanda begs Doogie not to operate on her. Doogie says Fowler will be doing the actual surgery -- he will only be assisting. Wanda says she doesn't want even want him to be there. Doogie says he has no choice -- he's the only other surgeon available.
During the surgery, Fowler is called away to work on a critical patient, forcing Doogie to finish the operation. Meanwhile, in the waiting room, Wanda's dad paces nervously, even as Doogie's dad tries to reassure them that Doogie is a first-rate doctor and that Wanda is in very good hands. Doogie enters and tells Wanda's parents that the surgery was successful. Wanda's dad grills him, asking if he did the entire procedure himself. Doogie says no, Dr. Fowler confirmed the diagnosis and helped with the surgery. Doogie says Wanda is in recovery, and they leave to go see her. Doogie leaves with his dad for home.
The next day, Dr. Canfield calls Doogie into his office, saying he caused all kinds of problems by admitting Wanda without her parents' consent, then not getting proper authorization to perform the surgery. Doogie says it was an emergency, and nobody else was available. Canfield says that if anything had gone wrong, the hospital could have been sued for millions. Doogie repeats that he had no other choice. Canfield says that the hospital pays out over $1 million annually in legal fees and malpractice claims. He says that it's not only his job to teach Doogie how to practice medicine, but also the consequences of practicing medicine.
Doogie visits Wanda, asking her if she's okay. She says she's fine, but when he asks to see her surgical dressing, she refuses, asking him to get someone else. Wanda says she doesn't want to see Doogie anymore as she is upset that he saw her the way he did, even though he didn't have a choice.
In the cafeteria, a depressed Doogie tells Jack and Nurse Curly what happened with Canfield and Wanda. Jack asks why Wanda doesn't want to see Doogie again. Doogie says that Wanda says it's too embarrassing, as her boyfriend was the one who examined her, even though he's a licensed doctor. Curly chides Jack over the last time he was examined by a doctor of the opposite sex. Scoffing, Jack tells Doogie to give Wanda time -- she'll get over it.
Later, Curly visits Wanda, saying that few people can brag about being operated on by a 16-year-old doctor. Wanda says it was also humiliating, as he was also her boyfriend. Curly says she knows it's none of her business, but she thinks Wanda is confusing embarrassment with anger and that she's lucky to have Doogie as a boyfriend.
When Doogie gets home, Vinnie needles him about operating on Wanda, but he brushes him off, saying he has to eat dinner and get back to the hospital. After dinner, Doogie tells his mom that he feels like he's being wrongly punished. Doogie's mom says that a pelvic examination is embarrassing to a 16-year-old girl, implying that emotionally, such an exam is like sexual assault. Doogie says that Wanda said she doesn't want to see him again. His mom says that Wanda probably didn't mean it, and the best thing he can do right now is to probably back off and wait for her to come to him.
That night, Doogie visits Wanda. She says that she didn't mean what she said about not wanting to see him again. Doogie says it's okay. Wanda asks him if he saw her naked, and he admits he did, but he didn't pay attention because he was too busy trying to be a doctor. Wanda kisses Doogie on the cheek, thanking him for taking care of her.
When Doogie gets home, he is writing in his computer diary when his dad enters. Doogie's dad says that no matter how prodigious his intellect, the conundrum of womanhood is seldom grasped and never fully understood. The conversation is cut short when Vinnie jumps in through Doogie's window. Doogie's dad exits, and Vinnie wisecracks about Doogie grabbing Wanda's "conundrum." Doogie responds that it was very mysterious. Saying he needs to get to sleep, he asks Vinnie to leave, which Vinnie does.
Doogie finishes his diary entry: "Went to see Batman. Parked at Mission Point. Removed Wanda's appendix. Broke up. Got back together. What can I say? Just your average weekend."
