In the E.R, Cameron says that the patient she is examining has an "aortic dissection", but continues to carry on a casual conversation with House, even stepping away to read him the file of another patient.
An aortic dissection is a severe medical emergency, which often requires immediate surgery. As a doctor, Cameron would have known this and would have never wasted time with idle conversation.
It makes no sense that Foreman demands House formalize his authority over the team. Foreman was hired by Cuddy. The logical thing would be for him to demand from Cuddy to formalize his authority over House's team.
