On January 28, 1986 the Shuttle Challenger broke apart just 73 seconds after launch and NASA lost seven astronauts. Investigations were carried out to discover what exactly went wrong during the launch, and in the preparations for the day that led to such a catastrophic failure, and resulted in the eventual termination of the Shuttle program.
