Sex & Nudity

  • All Necromorphs are naked. Despite this, they're so heavily mutated that their genitalia has practically disappeared, and they reproduce by infecting other humans meaning they don't need these anyway.

Violence & Gore

  • Like the original, this game is extremely gory and graphic.
  • "Dead Space" is known as one of the most violent video games ever created, featuring lots of extremely gruesome body horror and repeated graphic gory violence. This game, however, takes full approach of the current generation's technical capabilities to apply it on every aspect of the aforementioned, allowing it to be explicitly shown in even greater, messier and much more brutal detail, not helped by the exponentially increased graphical fidelity.
  • As with the original games, a ton of violence occurs here. Throughout this game, Isaac Clarke must kill horrific creatures known as the Necromorphs, extremely vicious undead monsters made from reanimated corpses that can only be killed via dismemberment and attack in an incredibly brutal, animalistic manner. Whenever a Necromorph is dismembered, gallons of blood and gore spray out all over the place in a gruesome fashion.
  • In addition to dismemberment, the game introduces the "Peeling" System, a mechanic in which weapons rip out layer-by-layer huge chunks of skin and flesh from the human body, allowing for exposed guts, bones, organs and more to be shown in extremely gory fashion.
  • As with the original, Isaac Clarke, the player character, can die in all sorts of extremely gruesome ways, including getting torn in half, decapitated, ripped apart, degloved, gutted and more shown in excruciating, in-your-face detail alongside absolutely grotesque death animations and hideously gruesome sound effects, all while a flatline sound is heard. It's all to the degree that each of these deaths is basically a "Mortal Kombat" Fatality.
  • On the environment, it's possible to find butchered human corpses in various states of horrific mutilation, some with exposed bones and spilled entrails.
  • The violence overall goes on par with stuff like "Dead Island 2" and "Terrifier 2", except without the black comedy of either and much more graphic.
  • The game's environment is very dark 90% of the time, which can hide some of the extremely brutal and realistic blood and gore. However, all weapons are equipped with a flashlight, and a good few areas are well lit, allowing the player to really see how graphic the game is.

Profanity

  • One thing to notice is that the profanity is much stronger than in the original. Words like "shit", "hell", "fuck", " damn" and others are used, but they're more commonly heard than in the original game and have a higher impact.
  • When Isaac either stomps enemies or runs out of bullets, he lets out a lot of F-bombs, multiple uses of "damn" and "shit", and some blasphemous exclamations. However, this is all justified in-game, as Isaac deals with a lot of physical and mental stress that only escalate and turn him into a battle-hardened individual, stopping at nothing until he accomplishes his mission.
  • Strong language throughout.

Alcohol, Drugs & Smoking

Frightening & Intense Scenes

  • If players so choose they can add a warning and a filter that hides and extremely disturbing scenes or warns players of what's going to happen, however with censoring it should be noted that while there is no visual, the sound is not affected
  • As a whole, this game is much scarier than the original, which speaks volumes considering that the original is known for being one of the scariest video games of all time. It's also a lot darker, nastier, and much more graphically violent and bloodier as well, with plenty of gruesome material and extreme violence portrayed in a disturbingly realistic manner. It's NOTHING compared to the original or any of its sequels, and it's clearly not for anyone under the age of 18.
  • The Necromorphs are extremely frightening and grotesque.
  • Many areas in the game's environment, be it the USG Ishimura or the Aegis VII, are covered in an extremely gruesome organic growth called "Corruption", with dozens of layers of bones, organs, veins, flesh, muscles and pustules fused together in one horrific biomass, and it also contains multiple corpses either strewn about, twisted in bizarre ways, or mutated into hideous grotesqueries forming part of said biomass. This can be highly disturbing and disgusting for some.

Frightening & Intense Scenes

  • As the game progresses, Isaac Clarke starts suffering from very disturbing hallucinations, which manifest in him hearing sinister-sounding voices and seeing nightmarish visions. His sanity erodes as well, growing more and more vicious, traumatized and deranged as a result of both of the Marker's influence and his horrific encounters with the Necromorphs.
  • As with the original, the game's semi-inanimate antagonist, The Red Marker, is an human replica of an alien artifact that causes severe insanity and drives people to suicide and murder. This can be seen happening multiple times in-game.
