- The final boss, while appearing to be a moon, is how players are to perceive death, implying that Sonic and Sage see something completely different. It is left unknown exactly what they see and that theme may be unsettling for some. The battle itself is also quite intense.
- The atmosphere and the music of this game may be intense and unsettling to players, especially during the Titan fights as very intense rock music plays during the battles.
- If this game were to be rated by the MPA it would be PG-13 for extended sequences of sci-fi violence and action.
- Sonic spaces out for a brief time due to cyber corruption, though this isn't too frighting.

Frightening & Intense Scenes
- Sonic begins to get sick from cyber corruption, and eventually it takes over his whole body. This scene could be somewhat upsetting for younger players, but it doesn't last long and it isn't dwelt upon.
- Sage dies at the end of the game after sacrificing herself for the sake of destroying The End, and the song that plays afterwards in the credits can be extremely emotional for some players.
- In the third DLC there is a new story, and this new content is incredibly challenging and could prove nearly impossible to younger players due to a boss rush with all of the previous Titans, and the final boss that heals itself for example.
