- Pete finds out that one of his old friends from the army is running a private clinic out of his house, even going so far as to perform surgery in a room in the back. When Pete explains it to Violet, she wonders whether his friend as a God complex and wants to have some control over a bad situation. When a girl is shot and is taken to the clinic, Pete realizes that his friend will not be able to help her in his house. He decides to make the 911 call. Pete doesn't call the cops on his friend just yet, but if he finds out that another clinic has been set up, Pete promises that he will. Pete realizes that he really wants to help people, so Violet suggests that he be able to take a few shifts in the emergency room at the hospital.
Meanwhile, the drunk driver responsible for killing Dell and hurting Maya arrives at Oceanside. He has come to apologize but Sam doesn't waste any time in trying to choke the man to death. The attack causes the man to go into cardiac arrest, but Sam doesn't seem like he cares very much.
There is a flashback to Sam's first major heart surgery, where the patient tells him that he needs to make a confession just in case he doesn't make it. The man, who is a teacher, admits to being a pedophile. When complications arise during the surgery, Sam doesn't take much initiative in saving the patient's life. He admits that he let the man die because of what he said. Naomi believes that this is where their marriage began to break apart.
Back in the present day, Sam notices an anomaly in the drunk driver's scan, but he's actively wondering whether he should tell anyone. Naomi tells Addison about what happened with the pedophile before talking to Sam before the surgery. Sam tells Naomi that the man has a hidden aneurysm and that Charlotte should be told about it.
Also Naomi talks to Addison about her relationship with Sam, and that this incident shouldn't be the beginning of the end of their relationship either. Meanwhile, Amelia returns to CA and asks for a job at Oceanside. She's a brilliant neurosurgeon after all, and Naomi can't argue with that.
Charlotte has been going on a house hunt, but she neglected to tell Cooper about it. She finally confesses that she can't stay in Cooper's current place because she has way too many bad memories associated with it. Cooper, however, can only think of the good times.
