Sex & Nudity

  • Players can encounter human corpses lying in the morgue: some are nude with fully visible male genitalia while other body parts are decapitated or dismembered (e.g., bloody torsos, severed arms)
  • None of the nudity is sexualised, however, it can be confronting if one isn't prepared for it (it's one of the few games to actually depict full frontal male nudity).

Violence & Gore

  • When players are attacked, splashes of blood appear onscreen.
  • Large bloodstains and pools of blood are also visible in some areas.
  • In the Justine DLC, the player is forced to solve puzzles or kill other characters to progress the game. The option to kill the helpless prisoners is presented as by far the easier option.
  • This can be seen as being morally questionable as the people you're killing are helpless to defend themselves and plead you to help them.

Profanity

  • On one of several occasions when Daniel mutters during the game he says: "That little b*tch."
  • The words sh*t, bastard and damn appear in the dialogue.
  • In the extra downloadable expansion "Justine," c*nt is said by one of the monsters populating the game, as it is a random line of dialogue it can potentially be heard an indefinite number of times by the player - but can also potentially be missed entirely

Alcohol, Drugs & Smoking

  • One the items players can use is Laudanum. Which is used as an health item.
  • An audio flashback occurs where people are unknowingly locked in a wine cellar. They drink a lot of the wine and it is implied they are getting drunk before they become aware that they are trapped in the cellar and the wine causes them to explode. You don't see this happen, but you see the aftermath when you enter the room. Not as graphic as it sounds.
  • Mild/infrequent. There are empty bottles.
  • Some of the 'low sanity' effects take the form of symptoms commonly associated with drug withdrawal and use, e.g. bugs crawling across the screen, walls shaking and bending, vivid hallucinations and occasionally seizures

Frightening & Intense Scenes

  • This game is known widely as the scariest video game ever made.
  • The game takes your mind and twists it into an entirely different perspective. Your character is mentally unstable, so when you witness disturbing events or spend too long in the dark you will begin to go insane, which will make the game even scarier.
  • The game is very dark, disturbing and intense.
  • Every time you witness something disturbing, you slowly start to go insane. The screen distorts and shakes, the music gets higher-pitched and you begin to hallucinate. The more disturbing things you've witnessed, the scarier the game gets.
  • The narration details things very dark and adult, and while not actually shown onscreen, strongly implies rape, torture, alchoholism and animal abuse. The aftermath of these events can be seen onscreen.
  • You cannot defend yourself from bad things. You do not have any weapons, so your only options are to run or hide, and neither of them are very effective.
  • Graphic descriptions of the workings of various real-life torture machines (i.e. the Judas cradle, brass bull, iron maiden, death by sawing) and audio of them being used on prisoners. No visual of these events is provided, but this audio is played when the player enters the room containing the torture device being used in the audio. This, combined with the overall atmosphere of the game makes this one of the most disturbing sequences in a video game.

Sex & Nudity

  • (When you finally reach his inner sanctum, Alexander is fully naked, and hovers above the players head; with his genitals in full view)

Violence & Gore

  • Not so much blood/gore but intense torture descriptions. Bunch of chopped up body parts (these can be fed to a water creature). Some blood through out the game. Creatures chase the main character. There's what looks like a dog's or wolf's head at one point in the game.
  • In one scene, you drill into a dead mans head and inject his blood into yourself. while you dont see the injection part, you do see a drill cut into flesh and blood flow out.
  • This includes lowering a large metal spike into a man's chest (can by viewed by the player and killing a man strapped to a rack. This cannot be viewed however).
  • It is revealed that your character, Daniel, and the antagonist have done some incredibly horrific things to prisoners (within the context of the plot, however it is meant to be seen as shocking and disturbing, as are most aspects of this game)

Frightening & Intense Scenes

  • The game gets increasingly disturbing towards the end of the game as you find out more about your character and what he's done.
  • There are monsters chasing you. They look very frightening but you can only see them if they get close to you.
