- Mrs. Snow walks through her school, locking up for the night. As she is completing her duties, she hears a noise from across the hallway. Upon calling out to whoever made the noise, she sees that it is serial killer Jay Sullivan, who has resurfaced after a year. Jay chases her through the school, trapping her in a classroom. When he finds her, he recounts a conversation they had when Jay was her student years ago, after which he brutally murdered a bully. Jay reveals that he knows Emily, his ex-Wife, is close by, and threatens Mrs. Snow, telling her to warn Emily, otherwise he will kill her and hang her body on the flagpole in front of the school.
Emily is sleeping in her bed, when a sound disturbs her. She sits up, seeing Jay in the doorway, tapping his knife on the counter. She closes her eyes, then opens them, seeing that Jay has disappeared. She looks around, when Jay appears behind her, trying to stab her. She evades him and tries to run away, but is stabbed by Trapper. She begs Jay to not kill her, but Jay chokes her to death after giving her the choice of being stabbed or choked. Emily wakes up from the nightmare screaming. She looks around the room again, panicked, making sure Jay is not really there.
Emily goes to see her therapist, Talia, who recommends that Emily lets go of Jay, and releases herself from the responsibility she has put on herself to save Jay. Meanwhile, at Caroline's house, Cora Fisher is seen washing up in the morning. Jay Sullivan breaks into the house and threatens Cora before killing her. Hearing the commotion, Caroline rushes in and tries to stop Jay from killing Cora. He grabs Caroline and throws her into the shower, locking her in the bathroom. He walks out of the bathroom, and walks down to the room at the end of the hallway, where his sister Jenny is sleeping. Sensing his presence, she sits up, horrified. He turns the light off, and Jenny is last seen screaming.
Aaron Burrows has been sent a photo of Cora's body. He calls Detective Jerry Biggs, and they formulate a plan. After their call, Aaron gets a call from an unknown number. Upon answering it, it is revealed to be Jay using a voice modulator. Jay taunts Aaron before hanging up on him. Frustrated, Aaron gets up and begins to walk away, before bumping into an unmasked Jay. Failing to recognize the serial killer, Aaron tells him to be careful, as there's a serial killer on the loose.
Thomas Fisher and Kennedy Rogers are in the break room of their precinct. Detective Gracie Kowalski, the daughter of the legendary Martin Kowalski, enters the room and consoles Thomas after Cora's death. It is revealed that Thomas was sent evidence of her murder. The three plan, and Gracie suggests getting Emily involved.
Jay sends Detective Jerry Biggs a video, taunting him. As Jerry and Aaron discuss what happened to Aaron, Jerry receives a lead from Darren Hardy. Aaron declines to join Jerry in following the lead. Jerry goes into the woods, searching for evidence of Jay being there. He runs into a nature photographer, but finds zero evidence of Jay ever being there. Jerry retreats to his home, where Emily meets him. She offers her help in killing Jay. Jerry is reluctant, but accepts her help, hoping to either find his missing daughter, Sophia, or gain closure on her murder.
Jay enters a therapy session with Joley, the Mother of Talia. She is cold towards him, revealing that she knows his true origins, and birth name. She warns Jay that she suspects him of the murders, and that if one more body turns up, she will turn him in, before kicking him out. As Jay walks out, Talia is walking in. She tries to take his hand, but he rejects her. Upset, Talia storms into the office as Jay leaves. Joley makes a tape, in case Jay kills her.
Some time later, Aaron goes into the Thompson Family Therapy Clinic, but finds Joley and Talia absent. He searches for Joley's notes on Jay, and finds them. He calls Jerry to inform him of this. As he is in the restroom, Jay breaks in, hoping to find Joley. He instead finds Aaron. He slams Aaron into the sink, breaking his nose. Aaron tries to crawl away. He tries to stand against a counter, but Jay starts to choke him. As Aaron fights back, Jay sees a knife, takes it, and stabs Aaron. Jerry walks into the clinic, after not hearing from Aaron for some time. He finds Aaron, feels a weak pulse, and calls for help.
Back at Jerry's home, Emily tries to console him, offering hope that Aaron may survive. They talk about Jay and his crimes, including how he possibly murdered Emily's Father. Jerry receives a call from Darren, who reveals that Martin has begun helping with the investigation, and has tracked down Sara Reid - Jay's ex lover, and the Mother of Jay's daughter. Emily asks to join Jerry in talking to Sara, and he reluctantly agrees. Meanwhile, Jay enters a captive room, where we see a dirty and bruised Jenny. She pleads with her brother, and asks him why he's doing this. Jay recounts the night their Father was killed, and the first time he himself killed. Jay leaves Jenny in the room, while she begs to be let go.
Having learned of Caroline's name, Jay returns to her home, where she is still locked in the bathroom. He tells her that he can't kill her, and demands that she leave. As she runs out, she tells him he needs help, to which he responds "I would get help if I didn't kill my goddamn therapist!". It is unknown who he is referring to.
Gracie, Thomas, and Ken discuss plans on their investigation. They go over notes given to them by Martin. Thomas suggests sending Darren Hardy and Vigo Johnson to investigate one of Jay's suspected hideouts. Ken reveals that he will be meeting with Jerry and Emily, and tells Thomas and a dejected Gracie to investigate another, less dangerous lead. Once Thomas and Gracie leave, Gracie tells Thomas that they will still help Ken, Jerry and Emily. She reveals that the location is Jay's childhood hideout. Thomas asks how she would know, and she tells him she'll tell him on the way.
Jerry and Emily arrives at Sara's house, where they meet her and Camille. Jerry interviews Sara, and Sara shows a knife that Jay left behind the last time they saw each other. Emily reveals that Jay has several personalities, and names Leslie, Dwight, and Angela as known names of some of his personalities. She also says he called himself The Harvester. Emily asks to meet Camille, and Sara allows it. Emily becomes emotional meeting her ex-husband's daughter.
Back at Jerry's house, Jerry and Emily meet with Ken, and the three plan to investigate one of Jay's possible hideouts. Emily and Jerry agree to kill Jay, while Ken insists that they will apprehend him.
Jay takes food to a captive Sophia Biggs, who begs to be released. Jay refuses, stating that Jerry will know it was him. He informs her that he will be moving her in an hour, and tells her to eat. He says that he didn't choose to become a killer, but was dealt a bad deck of cards. Jay visibly struggles with himself.
After another murder, Jay is seen outside of Jerry's home, looking at a photo of him, Emily, and Jenny. He is prepared to turn himself in, when Trapper finds him. Jay throws his mask to the ground, declaring that he is returning home to Emily and leaving the cult. Trapper threatens him, and they have a brutal fight. As Jay is about to win, Trapper rakes his eyes and chokes him out. As Jay lays on the ground, struggling to stay awake, Trapper steals the photo and rips it in half. He stabs Jay. He grabs Jay's mask, and says he will make sure that Jenny receives it, before stabbing him again, and leaving him for dead. Jay hallucinates that Emily is by his side. She degrades him, and demands that he finish what he started. Jay wakes up in pain, sees the half of the photo with Emily on it, and throws it away.
Jay returns to transfer Sophia to another location. As they walk in the woods, Jay is confronted by bounty hunters Jebediah Blackhawk and Snake Williams Jr. Sophia hides behind Jay, who threatens them to walk away, as he does not want to spill blood in front of Sophia. Another brutal fight ensues. Jay knocks out Snake, and locks up with Jebediah, who punches him where he was stabbed. Though temporarily stunned, Jay pokes him thumbs into Jebediah's eyes and knocks him out with a headbutt. Snake, having regained consciousness, sees a tree limb, grabs it, and breaks it over Jay's back, incapacitating him. He begins to choke Jay, but Sophia stabs him. Jay headbutts him, knocking him out. Sophia asks if he is okay, but Jay declines to answer. He leads Sophia into another room, where Jenny is seated. She is visibly cleaner than her last appearance. Jay declares that a storm is coming. Sophia and Jenny talk. Sophia says that Jay saved her life, and she does not see him as a monster.
Darren Hardy investigates the woods, but is not joined by Vigo Johnson. As he is on the phone, he spots Jay carrying a body. He tries to talk to Jay, who reveals that he is in the killer persona. He says that Darren needs him, and implores him to join him. They have a fight, but Darren is quickly incapacitated. Jay tells him to give Jerry his location, and vows that he will still be there, as he has unfinished business with Emily.
Jay returns to the room holding Sophia and Jenny, only to find that Jenny is no longer there. He asks Sophia where she went, but she says she doesn't know. Angry, Jay throws a chair, demanding to know where Jenny went, but Sophia says she must have left when they were asleep. Jay contemplates killing Sophia.
Jerry, Ken, and Emily arrive in the woods where Darren fought Jay. As they search, Jerry sees movement further in the woods, while Emily sees movement in the cabin. Ken and Jerry decide to split up to investigate the woods, telling a dejected Emily to investigate the cabin. Reluctantly, she agrees.
Jerry and Ken walk further into the woods where Jerry saw the movement, but see nothing. Jay sneaks up on Ken and stabs him to death after he tries to shoot him. Jay and Jerry have a fight, but Jay is weak because of his stab wounds from Trapper. Seeing this, Jerry kicks him where he was stabbed, and knees him in the head, knocking him out. Jerry runs back towards the cabin, leaving the gun behind. Meanwhile, Emily enters the cabin and sees Sophia. As they exit the cabin, Sophia notes that it is unusually quiet. Jerry runs up to them, and sees his daughter for the first time in a year. Emotional, they call out for each other, but Jay appears, stabbing Jerry multiple times. Sophia, seeing Jay as a monster, tries to fight him, but is blocked by Emily, who tells her to run and get help. A weakened and exhausted Jay confronts and taunts Emily, who tells him to kill her already. Jay tries to stab her, but cannot bring himself to do so. An emotional Emily screams at Jay, and stabs him in the chest. Jay falls to the ground, and comes out of his killer persona. He says that he is scared, as Emily holds his hand. As he seemingly dies, Emily puts her head on his chest.
Emily returns home, and has a mental breakdown, hallucinating Jay. Thinking he is really there, she punches at him, but finds that she was hallucinating. A news report comes on, confirming the deaths of Jay and Ken, while saying that Jerry is alive but in critical condition and fighting for his life. It also reveals that Sophia was found alive and is undergoing evaluation. Emily's friend, JoAnn, walks in, scaring Emily. They talk about Jay. As JoAnn is about to leave, Emily's phone rings, and a distorted voice taunts her. JoAnn takes the phone and talks to the caller, who eventually claims to be Jay. The caller claims to be in the house. Emily tries to leave, but JoAnn insists they investigate to determine the validity of the caller's claim. After finding no one, they turn around and bump into Jenny, who has returned home. JoAnn leaves, revealing she needs to meet with Jebediah and Snake. Jenny and Emily mourn Jay. Outside, JoAnn walks away from Emily's house, when she is met by Jay, who is alive. He asks her if Emily suspects her, and she says she doesn't. Jay says they will proceed with "phase two" in a few months. They both walk away.
Jenny is seen walking towards one of Jay's masks, which was placed by Trapper, she picks it up and puts it on. Some time later, Jebediah and Snake are on a road trip. After a bathroom break, they find that their hood is popped open and the battery cable was cut. Jenny sneaks up on them, and kills Snake. She then stabs Jebediah, who recognizes her as he dies.
Gracie and Thomas are seen investigating the woods, having received a text that they needed to see new evidence. Darren, who has taken Jay's primary mask, kills Thomas, and chokes Gracie unconscious, declaring that she won't survive long enough for "his sequel".
