In episode #3.5, a location is given to set the scene, aboard the salvaged Martian gunship. Prior to the events in 'Triple Point', the ship's transponder ID was changed from Rocinante to Pinus Contorta, but the location details say the scene is aboard the Rocinante, which technically no longer exists.
The young male officer rescued off of a Martian gunship by the Rocinante crew has several wounds and abrasions on his face. However, when he first appears, his wounds are mild, but when he and his compatriots attempt to take control of the Rocinante by force, his facial wounds appear much more severe, without explanation. Later, when he is reunited with the Martian fleet and meets the captain of the MRC Hammurati, the pattern and severity of his wounds changes again.
Gravity in all scenes on Io is too high. In reality, Io's surface gravity of 0.183 G is only slightly higher than that of Earth's moon Luna of 0.165 G.
