Lt. Casey is promoted to Captain and assumes a non-existent position of being in command of all three shifts for truck 81. Since there was no Captain already doing this, unless there was a new position created, there would be nowhere for him to be promoted. In reality, he would have to wait for a Captain opening and then be transferred to that position. This is a common error that takes place in TV public safety shows. For example, on In the Heat of the Night (1988), several characters were routinely promoted even though there was no one already in that position.
Gabby when extricating the man from the car that was holding up opened the door, which weakened the car's structure. All she needed to do was break the window, which would have allowed her to extricate the man and at the same time let the car's door remain intact to support the building.
