If someone eats a diet of 5000 kcal a day, it does not really matter what they eat, they will increase a lot of weight. Also, they will have some health troubles. In that sense I always thought that the concept of this film was unconvincing. It is a documentary in the same way that "Jackass the film" is a documentary about the dangers of doing stupid things.
What is presented here certainly isn't a scientific experiment. You cannot prove a health claim with a single example. It is implied that he was in good shape at the outset, but got problems with his liver during the experiment. However, in 2017 he has been reported as having stated: "I haven't been sober for more than a week in 30 years..." which would imply he was an alcoholic while filming this documentary. Thus I do not think we can trust that this film was created in a truthful manner, anyway.
I think this film will confirm the viewers' bias against McDonalds, in case they have such a bias (many people have). Yet it only proves that "too much of a good thing is bad", once more. This film was a forerunner of modern activism-journalism which serves the viewers' biases. People who watch this film will have made up their opinions before watching it, or they won't watch it.
It's a bit like those anti-drug films telling you how bad drugs are.
By the way, the vegan girlfriend in the film has since changed back to a non-vegan diet for health reasons... not sure if that includes McDonalds, though.

I generally like Kitanos films and watched this one for the sake of completeness.
I was very surprised at what I saw. This is slapstick humor, Benny Hill type. There are a few good jokes in here, let us say 5 minutes out of a total duration of 105 minutes. Some of the typical dramatical and visual elements of Kitanos films can be found here, maybe it was an experiment at filming slapstick in Kitano-style, along the lines of "it seemed like a good idea at the time"? Not my cup of tea, I was bored most of the time (that would be the other 100 minutes).
For some reason, the cautious comments that could have warned me come at the bottom of the list here in IMDb.
