SPOILER: Honestly, I loved it. I loved not knowing exactly what was going on... I kind of thing Sam was dead and found himself in some sort of afterlife, paying for something he'd done back when he was alive but without actually realizing what it was. (Killing the little girl, maybe even being abusive to his own family or a child molester, since they call him that several times)
The other idea I really liked while reading the reviews was that the little girl and his family were dead and he was sort of imagining everything else (The radio Host being himself, the voices being fake audience, the insults on his pager, etc) but the people chasing him were actually real, trying to somehow make him pay for what he did. I don't think I explained it very well but I think it's a really, really good movie. I enjoyed it a lot.
