The episode has a change of scenery, which didn't happen very often, in the Scrubs universe. Beside that, it's nothing special. As part one of a two-parter, it feels more like one big setup. The "story" is about arriving at the island, and cooking up the conflicts for part two. The jokes aren't that great, either.
